Support for SWIS at VTPBIS and Non-PBIS Schools

The VTPBIS State Team provides SWIS facilitation to all Vermont schools interested in obtaining the applications within the SWIS suite.

How to Obtain SWIS in Vermont

VTPBIS Schools:

Technical assistance on how to obtain SWIS is provided at PBIS training and written guidance is provided on the website – www.pbisvermont.org. Further, VTPBIS regional TAs and coaches can provide support for readiness.

Non-PBIS Schools:

All Vermont schools can visit the website – www.pbisvermont.org for information on how to obtain SWIS. For any SWIS Suite application, schools must complete and send the appropriate SWIS readiness checklist to the SWIS facilitator prior to approving a SWIS license for the school.   Once approved and licensed, schools can access online video tutorials at www.pbisapps.org or attend VTPBIS SWIS webinars. Further technical assistance is only available on a fee for service basis.

The School-Wide Information System (SWIS) is a web-based information system designed to help school personnel to use office referral data to design school-wide and individual student interventions.

For schools implementing at the Universal Level:

To obtain SWIS, please complete the following steps:

1) Complete the SWIS License Agreement and School Information Form and fax page 6(school information form) to Anne Dubie at (802) 656-1357. IMPORTANT: This document MUST be typed and page 6 MUST be signed by the Building Administrator as the Licensee.

2) If you are not a PBIS School, please complete the SWIS readiness checklist

3) Please put Sherry Schoenberg as your SWIS Facilitator and feel free to e-mail Anne ([email protected]) with any questions you may have.

4) Complete the SWIS User Personnel Information Form and fax this to Anne Dubie at (802) 656-1357.

Once we receive your forms, we’ll fax them to SWIS at the University of Oregon. We will contact you as soon as your account has been activated (usually within 2 weeks).

For schools implementing at the Targeted Level:

To obtain CICO-SWIS, please complete the following steps:

1) Complete the attached SWIS License Agreement and School Information Form and fax page 6 (school information form) to Anne Dubie at (802) 656-1357. IMPORTANT: This document MUST be typed and page 6 MUST be signed by the Building Administrator as the Licensee.

2) Please put Sherry Schoenberg as your SWIS Facilitator but feel free to e-mail Anne-Marie Dubie ([email protected]) with any questions you may have.

3) Complete the SWIS-CICO Readiness Checklist and fax page 1 to Anne Dubie at (802) 656-1357 and attach a copy of your Daily Progress Report (DPR).

Once we receive your forms, we’ll fax them to SWIS at the University of Oregon. We will contact you as soon as your account has been activated (usually within 2 weeks).

To learn how to use CICO-SWIS, please complete the following steps:

1) Complete the instructions above to add CICO to your SWIS Account.

2) Download and review the following resources:

3) (Optional) E-mail Anne-Marie Dubie at [email protected] with any remaining questions you have.

4) (Optional) Request a virtual SWIS-CICO consultation session to have your questions answered.

ISIS-SWIS INFORMATION – NEW!:

To be considered for a ISIS-SWIS license, schools must:

1) Be implementing PBIS at the Targeted Level;

2) Actively administering Functional Behavior Assessments (FBA) and Behavior Support Plans (BSP);

3) Contact your VTPBIS State TA to discuss appropriateness of ISIS-SWIS for their school

To obtain ISIS-SWIS, please complete the following steps:

1) Complete the ISIS-SWIS Readiness Checklist

2) Complete the SWIS License Agreement and School Information Form and fax page 6(school information form) to Anne Dubie at (802) 656-1357. IMPORTANT: The School Information Form must be typed and Page 6 MUST be signed by the Building Administrator as the Licensee.

3) Please put Sherry Schoenberg as your SWIS Facilitator but feel free to e-mail Anne-Marie Dubie ([email protected]) with any questions you may have.
